The Arizona Space Grant Consortium (AZSGC) is committed to creating a positive, inclusive, and engaging STEM experience for all of our participants. If an intern, ASCEND scholar, fellow, or mentor (past or present) needs assistance or would like to communicate an issue to the Space Grant Management Team, we’re here to support you.
We strongly encourage all Space Grant affiliates to have open and honest communication between all parties involved in issues that arise during the year. However, in the event that an issue continues unresolved and you would like to disclose information to the Space Grant Management Team at your local university (or the lead institution, University of Arizona, for Community College members), please fill out this form.
Purpose:
The purpose of this form is to gather information, look into comments/complaints/issues, and ultimately improve and maintain program integrity and overall participant experience. Information from these forms can be submitted anonymously, but details will be viewed by your local Space Grant Management Team or the lead institution (U of A) for Community College affiliates. To view your local Management Team, click here. Your submission will not be shared beyond your local Space Grant Management Team.
The intention behind distributing this information to your local university or lead institution is to allow the Space Grant Management Team to follow-up and resolve issues occurring at their site. If you choose to submit this form with your contact information, the Space Grant Management Team may follow-up with you on a more personal level to help resolve an issue.
We understand that this may be uncomfortable, but through this form and review process, we hope to gain affiliate confidence and trust, and preserve and maintain open and honest communication to better serve our program participants.
AZSGC primarily serves University and Community Colleges, and thus our affiliate institutions abide by local established policies and procedures. Please be aware that some comments/complaints/issues may also need to be addressed by your local institution.
